Do patients need to create a login to fill out digital forms?
No, they’ll only need a birth date and access code.
How long does it take for the patient to receive the intake email/text after I’ve sent it?
Patients should receive the digital intake form within less than 30 seconds. If they forget to complete the forms ahead of their appointment or time is of the essence, use in-clinic Kiosk mode.
How do I know when a form has been completed?
When the clinic opens, staff may decide to review completed intakes by filtering by eval date on the intake form page. Additionally, an email alert can also be set up so front desk staff receive notifications immediately upon a patient completing their intake paperwork.
Can we add our own forms?
Currently, we accept a maximum of four consent-based forms per organization.
